During this investigation we identified the impacted information and to whom the information belongs. The impacted system was an archived file not part of our active software.
What Information Was Involved
The investigation determined the information impacted may have included your name, address, medical information or health insurance information that could have included insurance and account balance information and medical procedure codes.

We take this incident and our obligation to safeguard patient information seriously. After discovering the incident, we took steps to secure our systems and notified law enforcement.
